Revive Ease Coaching Policy for Patients

 

1. Role of the Coach

Coaches may include medical assistants, therapists, therapy assistants, nursing assistants, or other supervised support staff working under the direction of a licensed healthcare provider, or in the future, digital/electronic assistants that provide automated guidance.
Regardless of format, coaching services are intended for pain coaching support only and are not a substitute for medical advice, psychotherapy, or behavioral health treatment.

2. Quick Summary – What to Expect

  • Coaches are allies and motivators, not medical providers.
  • They connect with patients through chat and scheduled calls, often monthly, depending on engagement and program goals.
  • Coaches communicate via the App’s chat feature and scheduled calls, typically responding within approx 48 hours.
  • Coaches will encourage and support self-management, but cannot provide medical advice, prescriptions, or emergency care.
  • You must contact your doctor for medical needs and call 911 (emergencies) or 988 (mental health crisis) if urgent.
  • Your logs and information are reviewed at set intervals, not continuously or in real time.

2A. Escalation & Safety Protocol

Coaches are not medical professionals and cannot provide emergency care. If you report concerning symptoms, your coach may advise you to seek care from your healthcare provider or call 911. Coaches are required to escalate safety concerns to supervising providers or clinic administrators as appropriate.

3. Your Responsibilities

Log activities, pain levels, and relevant data.
Communicate honestly with your coach.
Complete recommended tasks and use available resources.
Respect coach boundaries and use chat for meaningful concerns.

3A. Code of Conduct

Both coaches and participants are expected to maintain respectful communication. Misuse of chat features, inappropriate conduct, or harassment may result in removal from the program.

4. Privacy and Data Use

Coaches document interactions for program tracking and reporting.
Only necessary information is shared with healthcare providers or payors as needed.
All data handling follows Revive’s Privacy Policy and HIPAA regulations.

5. Program Graduation and Long-Term Support

When you achieve stable pain levels and functionality, your coach will prepare you for independent self-management.

6. Acknowledgment and Agreement

By using coaching services, you agree to:
The coach’s role and responsibilities.
Your own responsibilities as a participant.
Data use as described in the Privacy Policy.
